It started with our dad
Just like so many of us, our dad was living with a chronic condition some distance away. He was tracking his health – blood pressure, glucose, heart rate, symptoms, medications. But my brothers and I had no way to know how he was really doing.
We wanted to help. To show we cared. To guide him when we could. But we were all flying blind between doctor visits.
So we started ContinuumCare.AI
And recruited a leadership team with similar stories.
Not just for my dad. For millions of families navigating the same worry, the same distance, the same desire to stay connected to someone’s health journey, to show love.

That's what we built toward. Not just tools for tracking health, but tools for sharing it — with the people whose love and support can make a measurable difference.
