Healthspan is measured in years lived in good functional health, without major disease or disability. Longer healthspans add years to independent aging at home

Health data is everywhere, but important health data for the elderly are not well utilized

EMR data is mostly for health systems. A lot happens between Doctor’s visits, but home health data is an afterthought. We correct this imbalance by helping people self-monitor and keep all their data

Collecting home data from devices, whether connected or not, should be easy for everyone

We made clever use of technologies that billions of consumers are already familiar with - like taking pictures with smartphones, sending messages on secure text messengers, using unconnected devices and Web Apps. Less dependency on Bluetooth pairing, API integrations, 3G, paperwork and mobile apps.

People are naturally good at caregiving. We are less good at caring for ourselves

Healthy aging at home thrives when there is a caregiving circle of inter-generational support and a willingness to accept help. It is not a solo endeavor. Data collected over time helps reduce guesswork and turns worry into action.

If you harness home health data to AI you will get better health care vs. people who do not have comprehensive home data 

Health AI is enabling a historic shift from dependance on the sick-care health system, to informed self-reliance at home.

For us, these beliefs began with caring for our parents

Our father, in his 90s, was lovingly cared for by our youngest brother and sister-in-law. We are three brothers and two of us live thousands of miles away. We too wanted to support them when we could and share what we knew from years in healthcare.

 

But how, was the question.  

 

Each morning Dad’s caregiver took his blood pressure, blood sugar, SpO2, temperature, noted how he felt and wrote it on a clip board. His meds and meals would be adjusted accordingly. The paperwork was filed away with his lab reports and prescriptions.

 

The stack of paper kept growing. But we never really understood how he was doing. So to see what might be going on we digitized the paperwork and charted his home health data. Then we went a step further: we skipped the paperwork altogether and took photos of the readouts with a smartphone and sent them via WhatsApp to the cloud where we digitized it automatically. We could now see his hemoglobin and kidney lab trends, his heart rhythm (ECG), fluctuations in weight and episodes of fatigue. With this in hand we discussed our concerns with his doctors.  We became his health advocates.

 

Hardly a week passed without hearing from friends who were experiencing similar circumstances. So we kept building the App, not just for monitoring health, but also for making it understandable. This became ContinuumCare.AI
